I UNDERSTAND THE FOLLOWING:

1. Students MUST register themselves for the process. Parents or guardians cannot complete the registration for them. This is a disclosure policy. By agreeing to this policy, you are stating that you have given permission to Baylor Fraternity & Sorority Life staff and the Student Activities staff to review your academic records provided by the University.

The Family Educational Rights and Privacy Act of 1974, also known as FERPA and/or The Buckley Amendment of 1974, as amended, grants students attending post-secondary institutions certain rights and privacies regarding their education records. By submitting this form, the student may consent to release his/her education records and/or other information to a third-party for a defined purpose.

This form is not a binding agreement to a chapter (fraternity or sorority), but a FERPA requirement for Baylor Fraternity and Sorority Life in order to verify academic eligibility and grant permission to check and release academic and additional information shared through registration to the Office of Student Activities, chapter presidents, scholarship officers, advisors, and Headquarters staff for the purpose of verifying membership eligibility requirements and chapter/community grade reports. This release will be in effect for as long as membership is maintained in the fraternity and sorority community. You must notify Student Activities to revoke this consent.

2. I understand that except for illness, an emergency, or situation receiving prior approval from the Baylor Panhellenic Advisor, I must participate in all formal recruitment activities. Failure to participate in all events or to follow the recruitment guidelines may result in my removal from the formal recruitment process.

3. I am responsible for all financial obligations of sorority membership.

4. I realize that by completing this form that I am under NO obligation to join a sorority.

5. I understand the $55.00 registration fee is a non-refundable fee.

6. I understand I cannot save my registration form and come back to complete it at another time. I must complete registration in one sitting.

7. I understand I can only submit one registration form. Please do NOT submit more than one registration.
